Electronic Signatures & Records Consent

By signing your enrollment packet on joinexpress.pillarapps.com, you agree to do business with us electronically and to receive records electronically. This consent is given under the federal Electronic Signatures in Global and National Commerce Act (ESIGN) and applicable state Uniform Electronic Transactions Act (UETA) laws.
What "electronic signature" means here
When you click "I agree and sign" and either type your name or draw it on the page, that constitutes your legal signature. It has the same effect as a handwritten signature on paper.
What we capture for each signature
- Your typed name or drawn signature image
- The exact document you saw, identified by SHA-256 hash
- Your IP address, browser, and timestamp
- Your explicit consent to sign electronically and acknowledgment that you read the document
A Certificate of Completion is appended to the signed PDF documenting all of the above. We retain this record for at least 7 years.
You can request paper copies
You can request a paper copy of any electronically signed document by emailing shana@cdlchiro.com. Standard paper-and-postage fees may apply.
You can withdraw consent
You can withdraw your consent to do business electronically by emailing doc@cdlchiro.com. Withdrawing consent does not invalidate signatures you have already given. Going forward, we may need to mail forms physically and processing time will be longer.
System requirements
You need a current web browser, the ability to view PDF documents, and an active email account where we can send you copies of signed records.
